Every line of the COD form is a field or a section you can switch on, switch off and move. Open COD Form → Form customization and scroll to Manage form fields. The preview on the right updates as you work, and nothing reaches your storefront until you press Save in the bar at the bottom.
On a new install the rows run in this order: Order Summary, Discount Codes, Address Section Header, First name, Last name, Phone, Address 1, Address 2, Country, Province, City, ZIP code, Email, Shipping Rates, Totals Summary, Customer Notes, Newsletter, Terms and the Purchase button. Discount Codes, Country, Province, Customer Notes and Terms are hidden until you switch them on, and any custom field you add goes to the end. Every field is one more thing between the customer and the order.
Drag the handle on the left of a row to move it. The order you set here is the order the customer sees, so you can put the shipping rates above the address, or move the order summary to the bottom.
Province is hidden by default. If you sell to countries where Shopify needs a state or region, the field manager shows a banner beginning Consider enabling the "Province / Region field" field, naming the countries in question and telling you to set it visible and required. Skip that banner and the order is still created, with a dash written into the province.
Country is also hidden by default, which is right for a single country store. Switch it on when you ship to several, then pick which countries the customer can choose from and which one is preselected.
Open a field to change its label, placeholder, icon and validation.
