Click a row in Manage form fields to open its settings. Most fields share the same set, so once you have edited one you can edit them all. Changes appear in the preview at once and go live when you press Save.
Field label is the text above the input, Placeholder is the grey hint inside it, and Prefix is a fixed piece of text at the start of the field. If you have hidden labels in the form style settings, the placeholder is all the customer has to go on, so write it as an instruction rather than an example.
Required field makes the field mandatory, and Error message is what the customer reads when they leave it empty. A cash on delivery order needs a phone number more than it needs an email, so think about which fields genuinely block the sale.
Enable length validation opens Character length range and Length error message, which is the usual way to enforce a national postal code or identity number format. The email field adds Enable email validation. The phone field always checks that the number has a plausible number of digits and shows its own error if it does not.
The phone field lets you pick between Show phone icon, Show country flag and Show nothing, and offers Show country calling code before number. The city field has Enable city autocomplete, which suggests city names for the selected country and region. The ZIP code field has Auto-fill state & city from postal code, which works for India only. Province can be shown as a dropdown with Display as a select list.
The Country field is different. It reads Available countries (from Markets) from your Shopify Markets, and you narrow that down under Select countries. Default selected country and Preselect country in form control what is chosen when the form opens, and None (let customer choose) leaves it blank with your own Dropdown placeholder text. If a country you picked has no shipping rate, the app warns with Action required: Missing shipping rates.

Fix the missing shipping rates warning before you go live, or those countries cannot order.
