Two settings decide how the form reaches your customer: the mode, which is whether the form opens in a window or sits on the page, and the placement, which is which pages it appears on. A third, optional step pins the button to an exact position in your theme. All of them are in the app, not in the theme editor.
Open COD Form → Form customization. The first card is Choose form mode with two options. Pop-up Form means "The form will open in a window after clicking the button", and it is the default. Embedded Form means "The form will be displayed on the page at all times". Pop-up keeps your product page as it is and adds one button. Embedded shows the whole form immediately.
Go to the Settings tab and find Form placement. Pick Product page and cart, Product page only or Cart only. In pop-up mode a further card appears, Product page, asking "After clicking the COD button on the product page, show" with two answers: Only product from the current page or Product from current page + products from cart. The first is the default and keeps the order to the single product the customer is looking at.
By default the app decides where to inject the button. If you want it in an exact spot, add the app block named Progus COD Button in your theme editor. It is available on product and cart templates and has no settings of its own: it only marks the position. The block does nothing alone, because the Progus COD Form app embed is what loads the code. Both have to be on.
Still under Settings, the card Hide Shopify buttons is applied only on pages where the COD form or button is active. It offers Hide "Buy now", which is on for new installs and hides the dynamic checkout buttons, plus Hide "Add to cart" and Hide "Checkout".
Use the form preview on the right of the Form customization tab to see the result before you save.
Selling only some products by cash on delivery? Product restrictions in Settings limit the form to chosen products or collections.
In pop-up mode, the sticky button keeps the call to action in view on mobile. The Button customization tab still opens in embedded mode, but its controls are replaced by an untranslated Polish notice offering to switch back to pop-up.
