The COD form has its own list of shipping rates, because it never reaches Shopify checkout and so never sees Shopify's rate calculation. Open COD Form → Form customization and click the Shipping Rates row in the field list. The card is titled Shipping rates with the note "Configure shipping rates for your form. All prices use your store currency."
On a new install the app creates a single custom rate called Free shipping priced at zero, so that the form has something to offer from the first minute. If you charge for delivery, this is the first thing to change. Leave it in place only if free delivery really is your offer.
Import from Shopify pulls in the zones and rates from your Shopify admin, after confirming that your own custom rates and per-rate settings are preserved. Imported rates carry the badge Shopify. Later on, Refresh from Shopify repeats the import. Carrier calculated services come across as an entry named Carrier-calculated at checkout priced at zero, because the form cannot ask a carrier for a live quote. On imported rates the name and the price fields are both locked, so you cannot price that entry yourself. Hide the imported entry and add a custom rate at the price you charge.
Add rate creates one from scratch. Each rate has a Rate name, an optional Rate description shown under the name on the storefront, and a Price. Show in form and Hide from form control whether the customer sees it at all, and you can drag the rows to set the order they appear in.
Under Display settings you can change the Section title the customer sees above the methods. The free shipping block has Enable free shipping, a Free shipping rate label and a Text for free shipping, which is what appears in place of a price. Set Minimum order value to make it conditional, and leave it empty to always offer it. Show only free shipping when threshold is met removes the paid options once the customer qualifies.
Selling to several countries? Check that each one you allow in the Country field has a rate, or those customers cannot complete the form.
Need a rate that only appears for heavy or expensive carts? Add conditions to it.
