Any rate in the COD form can be given conditions, so it only appears when the cart matches. Expand the rate in Shipping rates and open Rate conditions. A rate with none carries the badge No conditions, always visible; a rate with some shows how many.
Add condition offers these types: Order total, Order weight, Quantity of products, Cart contains product, Cart does not contain product, Shipping city is one of, Shipping city is not one of, Province is one of and Province is not one of. The numeric ones take the operators is greater than or equal to and is less than, and weight can be given in kg or g. Read is less than as less than or equal to: a rate set to less than 100 also appears at exactly 100. Each condition can be tied to a single Country, or left on Any country.
Once a group holds two conditions, a selector appears above them offering Match all of the following or Match any of the following. With a single condition there is nothing to combine, so the selector stays hidden. For anything more complex, Add OR group creates a second block that uses the same selector, and a further one appears above both groups to say how the groups themselves combine. Despite the button's name, that selector also starts on Match all of the following, so every group has to match until you change it.
Each rate has a switch called When this rate is available, hide all others, with the hint "Useful for free shipping above a threshold". A rate with it on carries the badge Exclusive: when its conditions are met, the form shows only that rate. Use it sparingly, since two exclusive rates matching at once is hard to reason about.
Build a cart just above and just below each threshold, open the form, and check the right options appear. It is the only reliable way to catch a threshold typed in the wrong unit.
For a simple free delivery threshold, the free shipping block on the same card is easier than a conditional rate.
Keep the number of rates low: a long list costs you completed orders.
If a rate never shows up, check its country setting first, then its conditions.
