All of the form styling lives in the app, not in your theme editor, which is why the Progus COD Form app embed has no settings of its own. Open COD Form → Form customization and find Adjust form style. The preview beside it shows the result immediately.
The Form style picker offers eight ready looks, each with a one line description: Default, Modern, Dark Mode, Professional, Luxury Gold, Fresh Mint, Tech Neon and Black Friday. Pick the closest one first and adjust from there, rather than building a look from scratch.
Advanced options opens the individual controls: Text color, Font size, Background color, Field icon background color, Border radius, Border width, Form border color and Shadow size. The app puts a note next to the background colour: "Important: Changing the form background color may negatively impact conversion rates." Treat that as a reason to keep the form close to your page background.
Hide field labels removes the text above each input and leaves only the placeholders. It makes the form shorter, which helps on phones, but the customer loses the label once they start typing. If you switch it on, go back over your placeholders so each one still says what the field is for.
Two settings apply only in pop-up mode. Show close button (X) is on by default and puts a close control on the window. Enable fullscreen on phones is off by default and makes the pop-up fill the screen on mobile, which is worth testing if your form is long. If you go too far, Reset to defaults puts the style back. That button only appears once you have changed one of the advanced controls, so on a preset you have not touched there is nothing to reset.
Style the button separately on the Button customization tab: it has its own colours, icon and animation.
Check the result on a phone as well as a desktop before you save.
