Progus COD Form does two separate jobs, and most support questions come from mixing them up. One half puts a short order form on your product and cart pages, so a customer places a cash on delivery order without going through Shopify checkout. The other half never shows a form: it decides whether Shopify's own Cash on Delivery payment method appears inside the normal checkout. The app navigation follows the same split: COD Form for the first, Shopify Checkout for the second.
On the dashboard this is the tile named COD Form, described as "Show the COD form on your product page so customers can place orders in just a few clicks". The customer sees a button, or the form itself, on the product page or in the cart. They type their address and phone, pick a shipping rate, and press the purchase button. The order lands in Shopify straight away, unpaid, with the payment gateway set to Cash on Delivery.
The second tile is COD in Shopify Checkout: "Decide when Cash on Delivery (COD) should be available based on the selected shipping rate in Shopify's native checkout". The app links your COD shipping rate and your manual Cash on Delivery payment method together. Picking the COD rate hides every other payment method, so card, Shop Pay and PayPal disappear for that customer, and picking any other rate hides Cash on Delivery. On top of that you can write visibility rules on country, cart value, products and more.
If you want the shortest possible path from product page to order, set up the COD form. If you are happy with Shopify checkout and only want control over when COD is offered, set up COD in Shopify Checkout. Both count against the same monthly COD order limit, although a checkout order only counts once COD in Shopify Checkout is activated. They react differently at that limit. The form stops rendering, while in checkout the link between the shipping rate and the payment method stops being enforced, so Cash on Delivery becomes payable on every rate. Your delivery rules keep running, so the COD rate stays hidden wherever you told it to be.
