Almost every missing form comes down to one of six settings. Work down the list in order, checking the storefront after each step in a private browser window so you are not reading a cached page.
In the theme editor, under app embeds, Progus COD Form has to be switched on and the theme saved. If it is off, the COD Form page shows the banner The COD Form app embed is currently disabled in your theme.
Open COD Form → Settings → Visibility and check Show the form on your store. While it is clear, the form is never visible.
Look at the dashboard. A warning banner titled You have reached your monthly limit of 60 COD orders, with your own figure in place of 60, means the form has stopped rendering for customers. Upgrade, or wait for the next month.
Still under Settings, check Form placement. If it is set to Cart only, no product page will show the form.
Product restrictions has three modes. On Only selected products or collections the form appears nowhere except your list, and on Exclude selected products or collections it disappears from that list. If you only picked collections, heed the app's warning: on the cart page collection membership cannot always be worked out, so add the products too.
Country restrictions with Enable the form only for selected countries switched on hides the form everywhere else. It reads the country of the storefront market the visitor is browsing, not their IP address, so on a single market store every visitor gets the same answer. A country the app cannot work out is treated as allowed and the form is shown.
